THE GAP MOST SINGAPOREANS DON'T KNOW THEY HAVE

Singapore has a world-class healthcare system and a robust CPF structure. So it's understandable that many residents assume they're reasonably covered. The reality is more uncomfortable.

According to the Life Insurance Association Singapore's 2022 Protection Gap Study conducted with Ernst & Young, there is a 74% gap in critical illness protection and a 21% gap in mortality protection among economically active adults in Singapore. In practical terms, most people are significantly underinsured for the scenarios that would hurt them most.

MediShield Life covers basic hospitalisation but not private hospital stays, not the full cost of critical illness treatment, and not the loss of income if you're unable to work for months or years. A 2024 initiative by AIA Singapore found that over 2 in 5 customers are not familiar with the details of their own insurance policies. WHY CANCER IS THE CENTREPIECE OF ANY PROTECTION CONVERSATION

Cancer accounts for almost 1 in 3 deaths in Singapore, making it the leading cause of mortality. According to the Singapore Cancer Registry's 2022 Annual Report, approximately 48 people are diagnosed every day and incidence has increased most rapidly in those aged 30 to 39.4

From an insurance claims perspective, cancer accounts for 73% of all critical illness claims and nearly 40% of death claims. This is why critical illness coverage is not optional in a complete protection plan it is the foundation.

Critically, MediShield Life and Integrated Shield Plans cover hospital bills and approved treatments from the Cancer Drug List but they do not provide a lump-sum payout upon diagnosis, and they do not replace lost income while you are unable to work. That gap requires a separate layer of critical illness coverage.

As of 2024, approximately 70% of Singapore residents are covered by Integrated Shield Plans but coverage levels vary significantly, and many people don't know their plan's actual limits until they file a claim.

Replaces your income if illness or injury prevents you from working. This is one of the most underinsured risks among working professionals in Singapore and one of the most financially devastating if not covered. Your ability to earn is your greatest asset for most of your working life.

Provides a lump-sum benefit if you're permanently unable to work, ensuring long-term care costs and daily living expenses are covered without exhausting family savings or retirement funds.

HOW MUCH COVERAGE DO YOU ACTUALLY NEED?

A commonly used rule of thumb is life insurance coverage of 9–12 times annual income. The LIA Singapore recommends critical illness coverage of at least 4 times annual income, with some advisers recommending up to 5 times income to account for the full recuperation period.

But the right number always depends on your specific situation. A 29-year-old with a mortgage, a young child, and a spouse who relies on their income has very different needs from a 52-year-old whose children are independent and whose home loan is nearly paid off. The assessment has to be personal.

In my experience, the most important starting point is identifying your actual coverage gaps your outstanding liabilities, your family's projected expenses, the income your dependants would lose, and what government schemes and employer benefits already provide. Only then does the right coverage become clear.

THE RIGHT TIME TO REVIEW YOUR PROTECTION

Major life events marriage, a new child, a property purchase, a salary increase, starting a business, or a change in health are all triggers to reassess whether your existing coverage still matches your current responsibilities. If you haven't reviewed your plan in more than two years, now is the time.

PROTECTION PLANNING ACROSS LIFE STAGES

In Your 20s

This is the most cost-effective time to get insured. Premiums are at their lowest, your health profile is typically at its best, and locking in coverage early gives you the longest runway and the most flexibility later. The LIA's 2022 study found that 83% of Singaporeans aged 20–24 lack sufficient critical illness coverage. Starting early is the single highest-impact decision you can make on this topic.

In Your 30s–40s

Your financial responsibilities are typically at their peak mortgage, young children, possibly ageing parents. Your coverage needs are at their highest, and the stakes of being underinsured are greatest. Cancer diagnoses among younger Singaporeans have been rising most rapidly in this decade. A full protection review is not optional; it is essential.

In Your 50s+

Protection planning shifts in focus ensuring your health coverage is robust as medical needs grow, reviewing whether your critical illness and hospitalisation plans still serve you well, and beginning to think about how your estate will transfer efficiently to the people you care about. Legacy and continuity become part of the conversation.
